I recently found an adult mole salamander also. The recommendations were all for a fully terrestrial set up. Other members used coco based substrates mixed with organic soil with no additives, or organic soil alone. I personally use a coco based substrate called plantation soil that comes in a brick that you mix with water and tropical moss that comes the same way layered about two inches deep. The salamander prefers the coco based substrate and I will be switching to this when I replace the substrate. Make sure any water you use for anything in the tank is free of chlorine or other chemicals. I also include a small water dish, make sure the salamander can get out so it cant drown. I tried adding a hide feature but the salamander just burrows in the substrate anyways so I removed it. Go here http://www.caudata.org/cc/ for additional information. Adding onto everything that has already been said I have two more suggestions. Make sure you have a small drainage layer so the bottom cannot get waterlogged.
Second suggestion is make sure there is proper ventilation for you tank (screen top). Keep one area drier (area opposite from the water dish usually) than the other so the salamander can choose what moist level is needed/wanted.
