Illness/Sickness: Help!!! Tail has been damaged and is not healing!

Hello, My Axolotl Freckles who is about 3 years old got a wound on her tail about half a year ago, this tail wound healed quickly and turned into a minor scar and I thought little of it. Until recently in the past three months this wound opened up again, since then the wound had gotten worse and symptoms that look something like ammonia burns have appeared all over her body despite the ammonia levels being fine. I am really worried and I don’t know what to do and would really appreciate some help. I don’t know what caused the wound to open up again and how to help it heal or why this injury has led to ammonia burn like symptoms. (Sorry for the long paragraph just really worried about her) here is some images:

because of how delicate the tail is (especially the tip) it isn't unusual for it to get damaged, because of where the damage is it is doubtful if your axolotl has even noticed.
if your water levels are correct then it has most likely been done by catching itself.
because of how axolotls heal it is more likely to be a new injury rather than a reopening of an old injury.

Thank you so much for your help, but it still Begs the question. what is causing the ammonia burn like symptoms despite the ammonia of his tank being within safe parameters?
 
what are your full water levels ie.. temperature, ph, ammonia, nitrites, nitrates, kh, gh.
add 2g/l non-iodised salt into the water (this is to offer some protection against ammonia spikes), swab areas of injury/skin lesions with potassium permanganate.
is the water oxygenated with an air-stone?
has she had any problems with fungus?
how is her appetite?
what does her diet consist off?
is there any change in behaviour/movements etc..
does she have a hide/cave if so how often is it cleaned out?
how often are water changes performed and how much water is replaced?
with ammonia levels being good the cause for skin issues could be a variety of different things, anything from a mold infection to a reaction to something in the water hence all the questions asked to try to narrow it down.

full water levels:
-Temp: 23C (I know its a little hot at the moment as i left the door closed but it typically sits at about 21C)
-Ph: 7.6
-Ammonia: 0
-Nitrite: (NO2-) 0
-Nitrate (NO3-) 5

-No problems with fungus
-Her appetite is fine she ate almost 6 meal worms yesterday. with her diet consisting of mealworms, crickets and some nutrient pellets.
-no cave though she does like to sit on quite a dirty rock i will clean
-No change in behaviour except she doesn't seem to be swimming that much.
-Water changes are done roughly every two-three weeks
- I don't have an air-stone, is that something I should look into?
- I will add salt asap
- I will also try swabbing with potassium permanganate though i am concerned that may change the tank's water levels. should i have any concerns?

oxygenation is very important for both axolotls and the biological filtration, an air-stone is a must have.
the permanganate needs to be applied out of water but any that gets into the water via transfer is negligible and although unlikely to cause issues maintain parameter monitoring.
the temperature is high even at 21°c, 15°c -18°c being ideal colder the better although no lower than 10°c.

    Dear All, I would appreciate some help identifying P. waltl disease and treatment. We received newts from Europe early November and a few maybe 3/70 had what it looked like lesions under the legs- at that time we thought maybe it was the stress of travel- now we think they probably had "red leg syndrome" (see picture). However a few weeks later other newts started to develop skin lesions (picture enclosed). The sender recommended to use sulfamerazine and we have treated them 2x and we are not sure they are all recovering. Does anyone have any experience with P. waltl diseases and could give some input on this? Any input would be greatly appreciated! Thank you.
