First time Axolotl owner, missing toes???

Hello!!! I'm new to this website, and a bit lost, anyways, I'm a first time Axolotl owner, and my dad used to run an aquarium store, so I do get help from him... THOUGH, Orca, my Axolotl, seems to loose parts of her toes on one limb from time to time, I can't seem to find the problem. Tempature seems good, she doesn't seem to have any infections or anything, though. And I'm not sure if I'll be able to find a vet in Brazil for amphibians or axolotols that are in a reasonable distance to drive without stressing Orca out too much, not sure what to do, she acts normally, and so on and so on. It just happens. Her toes grow back, then after awhile, stubs again, grow back, takes awhile, stubs again. (Btw Im aware the tank is a bit dirty,I gotta clean it again. Usually once a month, from what Ive read?)

Very nice aquarium.
Nothing obviously problematic on the photos (4 toes on forelimbs, 5 toes on hindlimbs, are the normal numbers).
Isn't there something in the sand which could hurt the toes ? (the green peebles for example)

Thank you!! My dad gave me some tips on how to set this type of suff up, he loves terrariums and aquariums, lol. But no green pebbles, bit of algee that fell off some plants I trimmed, which Ill be siphoning up later, But I don't think so, it's just the gravel that slowly surfaced up from underneath the sand. Nothing sharp in the aquarium, either.

Unrelated to the missing toes, but it might be a good idea to take the gravel out. Generally if it fits in their mouth it’ll wind up in their mouth. Gravel can cause impaction
